Section 2B: Acknowledgement and Acceptance

I certify that I have been provided with training via the Carlisle Homes OHS Induction Booklet and at the in-person Carlisle OHS Induction Session. I have been issued with a digital copy of the OHS booklet and can confirm that all employees, contractors and agents whom I am responsible for who will work on a Carlisle Homes job site will read and sign off on the induction register provided within the OHS booklet and will act in accordance with the compliance requirements when working on Carlisle Homes job sites.

I understand that my staff and I must product the OHS booklet when working on site, when requested to do so by a Carlisle Homes representative.
